 Immunization FormChildren's Immunization Clinic The clinic is held on the second Monday of each month from 8:00 a.m. to 5:00 p.m. for children birth to 18 years old. Adult Immunization Clinic The clinic is held on the third Tuesday of each month from 8:00 a.m. to Noon. Appointments The immunization clinics are held in the lower level of the hospital in Hebron.
